How do I obtain a Medical Certificate?

For the first seven days absence from work a self-certification form should be completed. Such forms are available from your employer, the surgery or online at www.hmrc.gov.uk/forms/sc2.pdf (or here, if self employed).

If a further certificate is needed after the first seven days you will need to make contact with your doctor. A telephone consultation may be sufficient.
