HOW TO REGISTER
Welcome to Ide Lane Surgery
We are pleased to accept registration requests from patients who are living within the area indicated in the map below:
See our practice boundary and check your postcode is within our catchment area. (please note: the boundary line is not 100% accurate, please contact the surgery for confirmation that your postcode is within our designated area).
You can register in person at the surgery at any time during surgery opening hours. You will be asked to complete a registration form and a brief questionnaire and provide identification. We may make an appointment for you to come in and meet your new doctor and for a new patient check.
When completing the registration forms you will need to know your NHS number and you will need to return the forms with photo ID and a utility bill showing your current address. You will also need to complete a brief medical questionnare in order to give the Doctors an idea of your previous medical history.

Please complete the Online Registration, and the relevant New Patient Questionnaires. Please send (or bring) the signed copies to the surgery. If sending electronically, please scan your signed copies.
Practice Area
The practice serves Exeter west of the river Exe and villages south and southwest; Alphington, Ide, Kennford, Kenn, Shillingford Abbot and Shillingford St George, Dunchideock, Clapham and Longdown (see above). If you move out of our practice area, it will not be possible for us to look after you.
